O objectivo deste estudo era identificar as causas da subnotificação de eventos adversos de medicamentos em hospitais nos Camarões. A nível estratégico, concentrámos o nosso trabalho em três pontos, nomeadamente: primeiro, a avaliação dos conhecimentos dos profissionais de saúde em farmacovigilância; este estudo revelou um nível insuficiente de conhecimentos em farmacovigilância entre as categorias de profissionais de saúde. Em seguida, a determinação das suas atitudes face às manifestações dos eventos adversos dos medicamentos; verificou-se que os profissionais de saúde tinham uma atitude positiva face às RAM nos hospitais, que a proporção de eventos adversos variava consoante o tipo de hospital e que o nível de notificação era o mesmo em todos os níveis de referência. Finalmente, foram identificados os factores que dificultam o funcionamento da farmacovigilância hospitalar.

L'obiettivo di questo studio è stato quello di identificare le cause della sotto-segnalazione degli eventi avversi da farmaci negli ospedali del Camerun. A livello strategico, il nostro lavoro si è concentrato su tre punti: in primo luogo, la valutazione delle conoscenze degli operatori sanitari in materia di farmacovigilanza; questo studio ha rivelato un livello insufficiente di conoscenze in materia di farmacovigilanza tra le categorie di operatori sanitari. Poi la determinazione del loro atteggiamento nei confronti delle manifestazioni di eventi avversi da farmaci; è emerso che gli operatori sanitari hanno un atteggiamento positivo nei confronti delle ADR negli ospedali, che la percentuale di eventi avversi varia a seconda del tipo di ospedale e che il livello di notifica è lo stesso in tutti i livelli di riferimento. Infine, sono stati identificati i fattori che ostacolano il funzionamento della farmacovigilanza ospedaliera.

El objetivo de este estudio era identificar las causas de la infranotificación de acontecimientos adversos a medicamentos en los hospitales de Camerún. En el plano estratégico, centramos nuestro trabajo en tres puntos, a saber: en primer lugar, la evaluación de los conocimientos de los profesionales sanitarios en farmacovigilancia; este estudio reveló un nivel insuficiente de conocimientos en farmacovigilancia entre las categorías de profesionales sanitarios. A continuación, la determinación de sus actitudes frente a las manifestaciones de acontecimientos adversos a los medicamentos; se puso de manifiesto que los profesionales sanitarios tenían una actitud positiva frente a las RAM en los hospitales, que la proporción de acontecimientos adversos variaba según el tipo de hospital y que el nivel de notificación era el mismo en todos los niveles de referencia. Por último, se identificaron los factores que dificultan el funcionamiento de la farmacovigilancia hospitalaria.

Das Ziel dieser Studie war die Ermittlung der Ursachen für die Unterberichterstattung über unerwünschte Arzneimittelereignisse in Krankenhäusern in Kamerun. Strategisch haben wir unsere Arbeit auf drei Punkte ausgerichtet: Erstens die Bewertung der Kenntnisse des Gesundheitspersonals im Bereich der Pharmakovigilanz; diese Studie ergab ein unzureichendes Niveau mit einer Heterogenität des Kenntnisstandes im Bereich der Pharmakovigilanz zwischen den verschiedenen Kategorien des Gesundheitspersonals. Anschließend wurde ihre Einstellung zu unerwünschten Arzneimittelereignissen (UAW) untersucht. Dabei zeigte sich, dass die Gesundheitsfachkräfte eine positive Einstellung zu UAW in Krankenhäusern hatten, dass der Anteil der UAW in den verschiedenen Krankenhaustypen unterschiedlich war und dass das Niveau der Meldungen in allen Bezugsgruppen gleich war. Schließlich wurden die Faktoren identifiziert, die das Funktionieren der Pharmakovigilanz in Krankenhäusern behindern.

The objective of this study was to identify the causes of under-reporting of adverse drug events in hospitals in Cameroon. From a strategic point of view, we focused our work on three points: firstly, the evaluation of the knowledge of health professionals in pharmacovigilance; this study revealed an insufficient level of knowledge in pharmacovigilance among the categories of health professionals. Then the determination of their attitudes towards the manifestations of adverse drug events; it emerged that health professionals had a positive attitude towards ADRs in hospitals, that the proportion of adverse events varied according to the type of hospital and that the level of notification was the same in all reference levels. Finally, the factors that hinder the functioning of pharmacovigilance in hospitals were identified.

Fifty nine years after independence and forty years after the Alma Ata Declaration, there is no law governing the practice of TRM in Cameroon. At the continental level, the African Union as well as the African Organization of Intellectual Property and World Health Organization have paid a particular attention to the question and there are many recommendations and Guidelines addressed to Member States to put them into practice. Despite these initiatives, TRM has continued to exist and is used by approximately 80% of the population. There are facts which explain why TRM continues to gain many grounds in Cameroon and in Africa in general but the most evident argument comes from the understanding of medicine or medication. In fact, the understanding of the nature of medicine entails three important aspects: that of medication, culture and economic aspects.
