Nous sommes un centre de référence pour les patients pédiatriques atteints de leucémie lymphoblastique aiguë (LLA) dans le centre de l'Afrique du Sud. Le profil démographique de nos patients est-il différent et, si oui, de quelle manière diffère-t-il de celui des patients à l'étranger et localement dans d'autres centres en Afrique du Sud ? Une analyse rétrospective de 133 enfants atteints de LAL a été réalisée afin d'évaluer les caractéristiques démographiques et les différentes variables cliniques influençant le pronostic des patients atteints de LAL dans le centre de l'Afrique du Sud. L'âge inférieur à 1 an présentait le plus mauvais taux de survie globale (valeur p : 0,0170). Plus de 80 % des patients étaient anémiques et/ou thrombocytopéniques au moment du diagnostic. La LAL de la série B (RR : 2,44) était associée à un taux de survie global plus court. Les taux de survie des patients pédiatriques atteints de LAL dans le centre de l'Afrique du Sud étaient inférieurs à ceux cités dans les pays développés. En essayant de déterminer quels risques et quelles variables pronostiques influencent l'évolution des leucémies infantiles, en particulier dans le centre de l'Afrique du Sud, nous espérons trouver un meilleur protocole de traitement qui conviendra aux patients leucémiques dans notre contexte.

Somos um centro de referência para doentes pediátricos com leucemia linfoblástica aguda (LLA) no centro da África do Sul. Será que o perfil demográfico dos nossos doentes é diferente e, em caso afirmativo, de que forma difere do perfil dos doentes no estrangeiro e noutros centros da África do Sul? Foi efectuada uma análise retrospetiva de 133 crianças com LLA para avaliar os dados demográficos e as diferentes variáveis clínicas que influenciam o prognóstico dos doentes com LLA no centro da África do Sul. A idade inferior a 1 ano registou a pior taxa de sobrevivência global (valor de p: 0,0170). Mais de 80% dos doentes eram anémicos e/ou trombocitopénicos aquando do diagnóstico. A ALL da série B (RR: 2,44) foi associada a uma taxa de sobrevivência global mais curta. As taxas de sobrevivência de doentes pediátricos com LLA no centro da África do Sul eram inferiores às citadas no mundo desenvolvido. Na tentativa de determinar quais as variáveis de risco e de prognóstico que influenciam o curso das leucemias infantis, particularmente na África do Sul central, esperamos encontrar um melhor protocolo de tratamento que se adeqúe aos doentes leucémicos no nosso meio.

Il profilo demografico dei nostri pazienti è diverso e, in caso affermativo, in che modo differisce da quello dei pazienti all'estero e di quelli locali di altri centri in Sudafrica? È stata condotta un'analisi retrospettiva di 133 bambini con ALL per valutare i dati demografici e le diverse variabili cliniche che influenzano la prognosi dei pazienti con ALL nel Sudafrica centrale. L'età inferiore a 1 anno ha fatto registrare il tasso di sopravvivenza globale peggiore (valore p: 0,0170). Oltre l'80% dei pazienti era anemico e/o trombocitopenico alla diagnosi. L'ALL di serie B (RR: 2,44) è stata associata a un tasso di sopravvivenza globale più breve. I tassi di sopravvivenza dei pazienti pediatrici affetti da ALL nel Sudafrica centrale erano inferiori a quelli dei paesi sviluppati. Nel tentativo di determinare quali rischi e quali variabili prognostiche influenzano il decorso delle leucemie infantili, in particolare nel Sudafrica centrale, speriamo di trovare un protocollo di trattamento migliore che si adatti ai pazienti leucemici nel nostro contesto.

La edad inferior a 1 año presentó la peor tasa de supervivencia global (valor de p: 0,0170). Más del 80% de los pacientes presentaban anemia y/o trombocitopenia en el momento del diagnóstico. La LLA de serie B (RR: 2,44) se asoció a una tasa de supervivencia global más corta. Las tasas de supervivencia de los pacientes pediátricos con LLA en Sudáfrica central fueron inferiores a las citadas en el mundo desarrollado. Al intentar determinar qué riesgos y variables pronósticas influyen en la evolución de las leucemias infantiles, especialmente en Sudáfrica central, esperamos encontrar un mejor protocolo de tratamiento que se adapte a los pacientes leucémicos de nuestro entorno.

Wir sind ein Überweisungszentrum für pädiatrische Patienten mit akuter lymphoblastischer Leukämie (ALL) im Zentrum Südafrikas. Unterscheidet sich das demografische Profil unserer Patienten, und wenn ja, in welcher Weise unterscheidet es sich von dem der Patienten im Ausland und in anderen Zentren in Südafrika? We are a referral centre for paediatric patients with acute lymphoblastic leukaemia (ALL) in central South Africa. Is the demographic profile of our patients different and, if so, in what manner does it differ from that of patients abroad as well as locally at other centres in South Africa? A retrospective analysis of 133 children with ALL was performed to evaluate the demographics and different clinical variables influencing prognosis of patients with ALL in central South Africa. Age below 1 year had the worst overall survival rate (p value: 0.0170). More than 80% of patients were anaemic and/or thrombocytopenic at diagnosis. B series ALL (RR: 2.44) was associated with a shorter overall survival rate. The survival rates of paediatric patients with ALL in central South Africa were less than that quoted from the developed world. In attempt to determine which risks as well as prognostic variables influence the course of childhood leukaemias, particularly in central South Africa, we hope to find a better treatment protocol that will suit leukaemic patients in our setting.
