Serological tests for HIV and HCV are based on the detection of antibodies present in the donor's blood. This is what is known as the immunological window, a period in which, even if infected, the individual's serology is negative. The aim was to carry out a comparative study with a critical analysis of the screening methodologies for HIV and HCV testing in donors at the Hematology and Hemotherapy Center of the State of Piauí - HEMOPI. The comparison between the methodologies showed that although the agreement between the HIV tests was 99.7% and that between the HCV tests was 99.8%, the two methodologies are complementary, since the Kappa coefficients were 0.25 (fair) and 0.50 (moderate) respectively. In this way, it can be inferred that, as there are still no serological tests on the world market with 100% sensitivity and specificity, an alternative proposed to guarantee transfusion safety is the association between complementary methodologies, as is currently suggested by the Ministry of Health, through Health Surveillance.Keywords: Hemotherapy, HIV, HCV, Serology, NAT test.

Dies ist das so genannte immunologische Fenster, ein Zeitraum, in dem die Serologie der Person, selbst wenn sie infiziert ist, negativ ist. Ziel war die Durchführung einer vergleichenden Studie mit einer kritischen Analyse der Screening-Methoden für HIV- und HCV-Tests bei Spendern im Zentrum für Hämatologie und Hämotherapie des Bundesstaates Piauí - HEMOPI. Der Vergleich zwischen den Methoden zeigte, dass die Übereinstimmung zwischen den HIV-Tests 99,7 % und zwischen den HCV-Tests 99,8 % beträgt, die beiden Methoden sich jedoch ergänzen, da die Kappa-Koeffizienten 0,25 (mittelmäßig) bzw. 0,50 (mäßig) betragen. Il confronto tra le metodologie ha dimostrato che, sebbene l'accordo tra i test per l'HIV sia stato del 99,7% e quello tra i test per l'HCV del 99,8%, le due metodologie sono complementari, poiché i coefficienti Kappa sono stati rispettivamente di 0,25 (discreto) e 0,50 (moderato). In questo modo, si può dedurre che, non esistendo ancora sul mercato mondiale test sierologici con sensibilità e specificità del 100%, un'alternativa proposta per garantire la sicurezza trasfusionale è l'associazione tra metodologie complementari, come attualmente suggerito dal Ministero della Salute, attraverso la Sorveglianza Sanitaria.

Cet ouvrage vise à apporter une contribution tant sur le plan théorique que sur le plan pratique. Sur un plan théorique, la contribution concerne la compréhension de l¿évolution des méthodes de calcul des coûts, de tarification et de financement hospitalier. Sur le plan pratique, à travers une recherche-action nous avons appliqué une démarche pour calculer les coûts des actes médicaux réalisés en oncologie médicale. Compte tenu des deux objectifs, elle a été nécessaire de mobiliser un cadre théorique et conceptuel important relatif au calcul des coûts, au financement et à la tarification hospitalière. Ce corpus en littérature riche et diversifié à faciliter par la suite l¿application d¿une démarche de calcul des couts. L'objectif est de contribuer dans la définition d¿une politique de tarification appropriée de l¿acte de chimiothérapie en oncologie médicale des CHU(Centre Hospitalo-Universitaire) marocains et dans le processus de budgétisation de la prise en charge des patients en oncologie médicale du CHU HII de Fès. Elle vise également à enrichir les études de coûts réalisées dans les pays à revenu faible à moyen, et ce en l¿absence d¿une comptabilité analytique hospitalière.
